  • Valid passport with 6 months validity remaining along with xerox of first five pages with stamps.
  • 3 application forms filled with black ink in capital letters.
  • 2 recent passport size photographs, full name should be printed on back side.
  • Post application along with self addressed, stamped and registered envelope. It should be addressed to Consular section at Embassy.
